- Referral letter from your GP (without this you cannot get your Medicare Rebate)

- Your Medicare Card

- Details of any private health insurance.

- Any hearing tests, blood tests or letters from other specialists about your condition that your GP has.

- Radiology: please bring the CD or the film bag, not just the report. Dr Amott usually needs to review the films themselves before providing an opinion about your care. She does not always have access to these images online, as privacy restrictions often limit this access to the requesting doctor only.
